Medical Information & Requirements


The School Health Department of Kuwait’s Ministry of Public Health requires for every student in Kuwait a complete physical examination file, and TB test. Additionally, students under five years of age are required to receive a BCG vaccination. These must be completed before your child attends school at ASK.

Only government clinics are allowed to do these screening tests.

Please arrange to take your child to the TB section of the School Health Department along with:

  • One (3 x 4 cm) photograph.
  • The original birth certificate.
  • A copy of and the original civil I.D of the student. (if available)
  • A copy of and the original immunization certificate.
  • A copy of the information pages of his/ her passport.

Your civil I.D. address will guide you to the relevant regional clinic. The following clinics are open Sun. through Thurs. from 7:15 a.m. to noon:

Statements such as “all inoculations taken on the responsibility of the parent” or "all inoculations taken" will not be accepted. Parents are required to provide proof of Polio, DPT, series, and two MMR injections.

Every student attending ASK must also complete a Student Health File and Medical Emergency Release forms for first aid treatment. These forms will be provided upon admission.

Students transferring from different schools in Kuwait, need to:

  1. Fill out an ASK health file.
  2. Bring their TB/ BCG card and physical fitness file from their previous school.
  3. Bring an updated vaccination record.
